Scheduling appointments:
• All services are by appointment only (we are unable to treat walk-ins or emergencies)
• Appointments are ONLY accepted between 2 PM - 5 PM on Thursdays (call 919-824-4672)
• Clinic hours are 1 PM - 5 PM on Sundays
•Returning patients may call 919.824.4672 Tuesday-Thursday during regular business hours to leave a message

**No-Show Policy**
- New patients who miss their first appointment without canceling at least 24 hours in advance will lose eligibility at the Mariam Clinic for 6 months
- Existing patients who miss two follow-up appointments without canceling at least 24 hours in advance will lose eligibility at the Mariam Clinic for 1 year
- Patients who miss an appointment with a free/discounted specialist referral will lose eligibility for all referrals for 1 year; Important information:
All new patients MUST bring:
1) 2006 W-2 form
2) verification of identity and residency in Wake or Durham County (such as a driver's license and utility bill dated within the previous 60 days)
3) $10 cash for lab and/or radiology services; Services offered:
• Acute care (for illness and injury)
• Short-term chronic care (for diabetes, high blood pressure, etc.)
• Physician and social service referrals
• Discounted lab and radiology services
• Psychiatry and counseling
• Interpreters (Spanish, Arabic, Hindi/Urdu, Bengali, Bosnian/Croatian/Serbian)
**Please note that we do NOT offer emergency or obstetric services; Eligibility requirements:
• Ages 18 and over
• Must not qualify for public or private insurance (i.e. Medicaid, Medicare, NC Health Choice, Veterans' Benefits)
• Must live or work in Wake or Durham County
• Household income no more than 200% of the poverty level
• At least one member of the household must be employed
